What to Expect After Lip Fillers in the First Week

So you finally did it - you got lip fillers. Whether this is your first time or your third touch-up, the days following your treatment can feel a little uncertain. Is this swelling normal? Should my lips feel this firm? Why do they look uneven right now?

Relax. Almost everything you're experiencing in that first week is completely expected.
Lip fillers can enhance shape, volume, and symmetry, but the first week is all about healing and letting the results settle. Here's a straightforward, day-by-day breakdown of what your lips are going through -  and what you should (and shouldn't) do about it.

Day 1–2: Swelling, Tenderness, and That "Overfilled" Look

The most common lip filler side effects show up immediately and peak within the first 24 to 48 hours. Expect noticeable swelling, some tenderness to the touch, and possibly a bit of bruising around the injection sites. Your lips may look much fuller than you intended,  don't panic. This is the swelling talking, not your final result.

Tiny red dots or track marks from the needle are normal and fade quickly. Avoid wearing lipstick or lip gloss for the first 24 hours, and skip the gym. Heat and increased blood flow can worsen swelling.

What helps: Apply a cold pack (wrapped in a cloth - never directly on skin) for 10-minute intervals. Sleep with your head slightly elevated.

Day 3–4: Bruising May Peak, But Swelling Starts to Settle

By day three, any bruising may look its worst before it begins to fade. This is normal. The hyaluronic acid filler is still integrating with your tissue, so your lips might feel slightly firm or lumpy when you press on them.

Avoid salty foods, alcohol, and blood-thinning supplements like fish oil or vitamin E - all of these can prolong bruising. Stay hydrated. Drink plenty of water, since hyaluronic acid lip filler literally attracts and binds water molecules, which helps the filler settle smoothly.

Day 5–6: Things Start Looking More Like "You"

Halfway through the first week, most patients notice the dramatic swelling has eased significantly. Your lips start to look more natural, and the result begins to resemble what you actually asked for. Unevenness from days one and two often self-corrects during this phase.

You can return to most normal activities. Light exercise is fine. However, continuing to avoid intense heat, saunas, hot yoga, and prolonged sun exposure can all cause the filler to break down faster over time.

Day 7: Your Lips Are Settling In

By the end of the first week, your lip filler results are much closer to their final appearance, though full settling can take up to two weeks. The firmness softens, bruising fades, and you can finally get a real sense of your new shape and volume.

If something still feels noticeably off,  significant asymmetry, a hard lump, or prolonged pain, contact your provider. Most reputable med spas offer a follow-up appointment around the two-week mark to assess your results.

What’s Normal vs. When to Check In

Normal:

  • Swelling, tenderness, mild bruising

  • Temporary asymmetry in the first few days

  • Small, soft lumps that improve over the week

Contact your provider if you notice:

  • Severe pain or worsening swelling after day 3

  • Skin color changes (pale, dusky, or bluish areas)

  • Signs of infection (increasing redness, warmth, fever)

  • Lumps that don’t improve or become hard/painful
